The region is divided into the four provinces of L'Aquila, Chieti, Pescara, and Teramo. Abruzzo is considered culturally, linguistically, historically, and economically a region of Southern Italy, although geographically it may also be considered part of Central Italy. Until 1963 it was part of the region Abruzzi and Molise. The term Abruzzi (in the plural form) derives from the time when the region was part of the Kingdom of the Two Sicilies and the territory was administered as Abruzzo Citra or Citeriore (Nearer Abruzzo) and Abruzzo Ultra or Ulteriore I and II (Farther Abruzzo I and II), that being nearer and farther from Naples, the capital of the kingdom. Abruzzo Citeriore is approximately present-day Chieti Province, Abruzzo Ulteriore I comprised the Pescara and Teramo provinces, and Abruzzo Ulteriore II is approximately nowadays the Province of L'Aquila.

Abruzzo stretches from the Apennines to the Adriatic Sea and includes mainly mountainous and wildland. The mountainous inland is occupied by a vast plateau including the massifs of Gran Sasso d'Italia (2912 m a.s.l., the highest mountain massif of the Apennines), Maiella (2793 m a.s.l.), and Sirente-Velino (2487 m a.s.l.). Abruzzo's topography is the result of the violent geological forces that have shaped the region for millions of years. Central Italy lies on two fault lines and at the edge of the Eurasian and African continental plates. The collision of these plates is the cause of the abrupt release of accumulated stress in catastrophic earthquakes that occurred in Abruzzo. The most recent example is the earthquake that devastated L'Aquila and the surrounding territory in April 2009 and the series of smaller quakes in 2016-17, although there have been many others, such as the terrible quakes of 1703 in L'Aquila and 1915 in Avezzano.

From a geological point of the view, the region almost completely consists of sedimentary rocks. In the west, the mountains of Abruzzo are primarily composed of limestone formed in a marine environment during the Pliocene and the Early Pleistocene. In the east, the land that slopes gently down towards the Adriatic Sea is composed of Cenozoic clay and limestone deposits.

Several small occurrences of ultrapotassic pyroclastic rocks of Pleistocene age, lying in the so-called Intra-Apennine Magmatic Province (which also embraces Umbria and part of Latium) crop out along the axial zones of the Apennine chain. On the contrary, the Eocene-Oligocene subvolcanic dyke swarm of Monte la Queglia shows a mafic-alkaline petrochemistry and is similar to the volcanic outcrops of Gargano Promontory (Apulia).
